Benefit & Pension Reciprocal Forms

If you are a member of another IUPAT local (i.e. your “Home Local” is outside BC) and would like your Pension contributions and Health & Welfare hours sent (reciprocated) to your home local, please review the instructions below and fill out the reciprocal forms by following the link at the bottom of this page.

1. We encourage you to confirm your membership status with your home local by reaching out to them directly:

**NOTE** If you are not a member in good standing of another IUPAT Local in Canada you will be required to sign up with DC38.

2. Filling Out The Forms

  • Before you can start, you will have to accept the terms and conditions.
  • The first page is the agreement for Health and Welfare; the second page is for Pension.
  • There are fields that cannot be filled out, such as your SIN. We will ask for this later if we don’t already have it.
  • Many of the fields will auto-populate once you start entering your information. For example, when you enter your home local, that local’s address information will be automatically entered into the corresponding boxes.

3. Submitting The Forms

  • Click submit and provide your email address.

4. Receiving Your Copy

  • Once you have completed the forms, DC38 will receive notification to finalize.
  • Once finalized, a copy will be automatically emailed to you.
